ED 315 PROFESSIONAL FIELD EXPERIENCES Evaluation by Cooperating Teacher Directions: Please note students strengths or areas in which s/he

can improve in each category below. When possible, please provide specific examples or comments to support your view. 1. Conducts self in a professional manner. (e.g., attendance, preparation, respect) Dionetta always attended when she was supposed to. Dionetta always had her work prepared and was ready to teach the students. She worked with individual students when she or I were done teaching. 2. Encourages development of a positive self-image in learners. (e. g., gets to know students, positive interactions with students) Almost immediately, Dionetta learned the students names and interacted with them in a positive manner. Dionetta would use down time to help students with their work or to encourage students who were not doing well. 3. Interacts effectively with students, colleagues, administrators, parents. Dionetta appeared to feel right at home. She asked questions, interacted with other staff, and worked with students with no hesitation. She will fit in well with any staff. 4. Demonstrates effective teaching skills at the field student level. (e.g., planning, implementing lesson) Dionetta did an excellent job creating her lessons and executing them. 5. Communicates effectively through speaking, writing, and listening modes. Dionetta has excellent communication skills. She can communicate with whoever she needs to (students, teachers, and administrators) Final comments/suggestions for student: One of my classes is particularly trying. I would have like to see Dionetta teach this class, just to see how she handled the stress. Overall Dionetta did a great job with this field placement.
