Any activity on or off campus presents concerns to you and to us. Be assured we will do everything possible to ensure a safe and enjoyable dance. PLEASE ARRIVE AT 11:00PM to pick up your students after the dance. The Winter Formal is a school-sponsored event and that all school rules and policies apply. Students must abide by the Fremont Union High School District Code of Conduct as representatives of the school and district. Administrators, teachers and security personnel will be present from its start to its conclusion. Anyone attending the dance that we determine to be under the observable influence of any controlled substance or exhibiting any inappropriate behavior will be detained until a parent is contacted to pick up the student. FORMAL BIDS ARE NOT TRANSFERABLE. This form is due at the purchase of the dance ticket.

I hereby grant permission for my student to attend the Cupertino High School Junior Prom. I understand that this is a school-sponsored event, and that any action by my son/daughter deemed inappropriate may be subject to disciplinary measures by the school and removal from the event. As stated in California Education Code Section 35330, I agree to hold harmless the Fremont Union High School District, its officers, agents and employees, from any and all liability or claims, which may arise out of, or occur, in connection with my students participation in this event. I hereby waive any and all claims which I may have, jointly or separately, against Cupertino High School, the Fremont Union High School District and/or the State of California and/or their officers, agents, or employees, for injury, accident, illness, or death occurring during, or by reason, of the Winter Formal.
