Dear Parent, Student, and Guest:


CCHS strives to provide a positive social atmosphere at dances and other events throughout the
school year. To promote an appropriate environment for students, the school has developed
dance behavior expectations, dress standards, and conduct guidelines. When purchasing dance
tickets, students are agreeing to abide by the behavior expectations, dance dress standards, and
dance conduct guidelines listed below. Please read before purchasing tickets or completing the
Guest Pass Agreement.
Please note that Calvary Chapel High School Administration must approve all
guests.

Dance Behavior Expectations


● No student will be admitted more than 60 minutes after the start of the dance unless
prior arrangements have been made with the ASB Director or CCHS Administration.
Students may not leave the dance any time before 30 minutes prior to the end of the
dance without prior consent arranged by the ASB Director or CCHS Administration.
● All students will be observed for use of illegal or controlled substances prior to the entry
to the dance. Security may conduct pat downs and purse searches. Any student who
refuses to comply will be denied entry and parents will be contacted.
● Any vehicle, including limousines, can be searched at any time. Student drivers and
passengers will be held responsible and will be given consequences for any illegal
substance in the vehicle.
● Any student exhibiting disruptive behavior, suspected influence of illegal or controlled
substances, positive breathalyzer test or failure to adhere to dance guidelines will be
removed from the dance and parents will be called to retrieve him/her. No refunds will
be given.
● Guests must be in high school or must have graduated high school within the last year.
Guest passes must be turned in and approved by the ASB Director prior to the purchase
of tickets for the upcoming dance. All Guest Passes are subject to review by
administration and may be denied. Students may invite a student from another school to
attend CCHS events.

Phone Policies
We desire to conduct school events for the 2022-2023 school year as wisely, safely, and fully as
possible. We trust that these policies will provide a framework for safety for our events:
● Cell phones will be collected at the entrance and safely stored for the duration of the
dance; phone access will be permitted for parent contact at any time throughout the
evening. The HS Office phone will be open and available if you need to reach your

Dance Dress Standards


While school dress standards apply to all school events, the following guidelines address
expectations specific to school dances. If the student is in violation of dance dress
standards at any time, the student will be immediately required to make necessary
adjustments to their attire or asked to leave the venue. Repeated violations may result in
the loss of the privilege to attend future dances.

Full-length dresses with straps (off-the-shoulder/single shoulder are allowed, no


see-through cutouts)*
Tuxedos / Suit and tie (bowties or ties and jacket)**

*Dress code specifications: Necklines on dresses may not plunge below the bust line; no
abdomen/torso exposed; slits may not extend above the fingertips; sheer fabric or lace
constitutes exposure of the skin

**Dress code specifications: No t-shirts, jeans, or sweats; no undergarments exposed;


expected attire: belts/suspenders with a shirt tucked in, tie, and jacket

NOTE: Students wishing to attend the dance wearing attire considered to fall outside
these guidelines should seek prior approval from the ASB Director.

Dance Conduct Guidelines:


These guidelines apply on and off the dance floor. Sexually suggestive dancing will result in the
consequences listed below. To be explicitly clear, students are expected to adhere to the
following guidelines:
● No grinding
● No bending over
● No straddling legs. Both feet must remain on the floor
● No touching of breasts, buttocks, genitals or pressing up against one another in a manner
which is inappropriate
● No “making out” (overt and/or prolonged public displays of affection)
● Hands on waists or shoulders only (depending on dance style)
● No moshing
● No crowd surfing
The above-mentioned behaviors and acts will not be tolerated and individuals, couples, or
groups engaging in such acts or movements will be removed from the dance and
parents/guardians will be contacted. School administration reserves the right to make decisions
on suitable dance movements and on student dress based on appropriateness and good taste.

Dance Consequences:
If at any time a student or their guest violates the Dance Behavior Expectations, Dress
Standards, or Conduct Guidelines, they may be removed from the venue and the behavior will be
documented for further discipline. If such actions are carried out, parents/guardians will be
notified and the student will be subject to further consequences. Repeated violations may result
in loss of privilege to attend dances in the future.

Please note that administration reserves the right to remove a student and/or their guest from
the dance without a warning if the violation warrants. NO REFUNDS WILL BE PROVIDED
FOR REMOVAL FROM THE DANCE.

PARTICIPATION AGREEMENT: I acknowledge that participation in the activity described


above involves risk to the participant (and to the participant's parents or guardians, if the
participant is a minor), and may result in various types of injury including, but not limited to,
the following: sickness, exposure to infectious/communicable disease, bodily injury, death,
emotional injury, personal injury, property damage, and financial damage.

In consideration for the opportunity to participate in the activity described above the participant
(or parent/guardian if the participant is a minor) acknowledges and accepts the risks of injury
associated with participation in and transportation to and from the activity. The participant (or
parent/guardian) accepts personal financial responsibility for any injury or other loss sustained
during the activity or during transportation to and from the activity, as well as for any medical
treatment rendered to the participant that is authorized by Calvary Chapel Schools or its agents,
employees, volunteers, or any other representatives (collectively referred to as Calvary Chapel
Schools). Further, the participant (or parent/guardian) releases and promises to indemnify,
defend, and hold harmless Calvary Chapel Schools for any injury arising directly or indirectly
out of the described activity or transportation to and from the activity, whether such injury
arises out of the negligence of Calvary Chapel Schools, the participant, or otherwise.
If a dispute over this agreement or any claim for damages arises, the participant (or
parent/guardian) agrees to resolve the matter through a mutually acceptable alternative dispute
resolution process. If the participant (or parent/guardian) and Calvary Chapel Schools cannot
agree upon such a process, the dispute will be submitted to a three-member arbitration panel for
resolution in accordance with the rules of the American Arbitration Association.

EMERGENCY PERMISSION: I/we the parents/guardians, do hereby authorize officials of


Calvary Chapel Schools to contact directly the persons named on this card, and do authorize the
named physician(s) to render such treatment as may be deemed necessary in an emergency, for
the health of the child. In the event the physician(s), other persons named on this card, or
parents cannot be contacted, the school officials are hereby authorized to take whatever action is
deemed necessary in their judgment, for the health of the aforesaid child.

AUTHORIZATION AND CONSENT TO TREATMENT OF MINOR: I/we the parents/guardians


do hereby authorize Calvary Chapel Schools as agents for the undersigned to consent to any
x-ray examination, anesthetic, medical or surgical diagnosis, treatment, and/or hospital care
which is deemed advisable by, and is to be rendered under the general or specific supervision of
any physician or surgeon licensed under the provisions of the Medical Practice Act on the
medical staff of the licensed hospital, whether such diagnosis or treatment is rendered at the
office of said physician or at said hospital. It is understood that this authorization is given in
advance of any specific diagnosis, treatment, or hospital care being required, but is given in
advance to provide authority and power on the part of the aforesaid agents. This authorization is
given pursuant to the provisions of Section 25.8 of the Civil Code of California. Authorization is
hereby given to school personnel to administer first–aid treatment during school activities or to
call the paramedics, or rescue squad, as deemed necessary.

PHOTOGRAPHY RELEASE/HANDBOOK AGREEMENT: I/we the parents/guardians do


hereby give permission to Calvary Chapel Schools to use my child’s picture, image, or likeness on
all Calvary Chapel Schools publications, including school websites and media presentations.
I/we the parents/ guardians, agree to follow all pertinent school policies, including those
relating to procedures and discipline which are outlined in the parent/student handbook. The
handbook can be found at www.calvaryschools.org. Parents are responsible to keep the office
informed of any change of address, phone numbers, employer information, and emergency
contacts.

I give my child permission to attend this event. I understand the statements above and support
the guidelines, expectations, and dress code that Calvary Chapel High School has provided (see
attached).
