Readmission Acceptance Form

I, vivek barman student of 3th

year seeking admission to Program , at ADYPU School of would like to express my gratitude to
Ajeenkya DY Patil University for granting me admission to AY 2023-24. I hereby undertake and state
as follows:

I, hereby state and affirm that I shall take full responsibility in respect of my conduct, discipline,
attendance and payment of fees and also my adherence to the following rules and regulations of
Ajeenkya DY Patil University (ADYPU) during the period of my undergoing the said course, and
that, in the event of any commission or omission on my part, the university shall be fully empowered
to take appropriate action against me including discontinuation from the Program. The rules &
regulations of Ajeenkya DY Patil University are as follows; which I have read, understood and agree
to abide by them.
1. Students enrolled at Ajeenkya DY Patil University are expected to conduct themselves
properly at all times. They are expected to observe high standards of behavior and integrity, which
will reflect favorably upon them, their families and Ajeenkya DY Patil University. They are expected
to abide by the laws of the city, state, and nation, and by all rules and regulations of Ajeenkya DY
Patil University. The student will not participate in any political or antisocial activity; otherwise the
student will be liable to be expelled from Ajeenkya DY Patil University. Insubordination and rude
language or unbecoming conduct is sufficient reason for the dismissal of the student.
2. It is the responsibility of each student to adhere to the principles of academic integrity.
Academic integrity means that a student is honest with himself / herself, fellow students, instructors
and Ajeenkya DY Patil University in matters concerning his or her educational endeavors. Thus, a
student should not falsely claim the work of another as one's own, or misrepresent himself / herself.
3. All tests, theory and practical examinations as specified in the course structure are
compulsory and every student has to appear for these examinations. It is mandatory for the students to
maintain minimum 80% attendance in all heads of study, studios, and related activities.
4. Office of Ajeenkya DY Patil University should be informed in writing of any change in
address of the student and parents on occurrence. The student should help in maintaining the buildings
and the campus clean and tidy.
5. No certificate or transcript of a record will be issued to a student who has not made a
satisfactory settlement with the Accounts Department of ADYPU for all indebtedness to Ajeenkya DY
Patil University. A student may not be permitted to attend classes or appear for assessments or
examinations after the due date of any unpaid obligation.
6. The student must form a habit of reading the Ajeenkya DY Patil University notice board
regularly. Similarly, a student must regularly read the emails sent on his / her official university
account. A notice displayed / email sent on official email id will be assumed as read by all students.
I further state and undertake that I am physically and mentally fit for undergoing the strenuous course
of theoretical inputs and practical classes essential for completion of the course for which admission
has been granted to me, and, during the course of undergoing practical demonstration or studios or
study tours or visits if any accident or mishap occurs due to my own misconduct or mala fide intent or
for any other unforeseen reasons resulting in any kind of injury to me or loss of limb or life, in such
events, I myself will be fully responsible for the resultant situation and I shall not hold Ajeenkya DY
Patil University responsible for the same in any manner, whatsoever.
I am aware that I may be required and compelled to leave the Ajeenkya DY Patil University at any
given time if my conduct is unsatisfactory, if my ability to do practical work proves unsatisfactory, if
my conduct is not proper, my academic performance is poor, if found to have deliberately damaged
property of Ajeenkya DY Patil University, if payment of fees and dues are not made within the
prescribed period, if found participating in agitation, political activities, or found acting in a
prejudicial manner towards Ajeenkya DY Patil University, if suitable response is not forthcoming
from my parent / guardian to communication regarding absence, misbehavior, poor performance or
non- payment of fees, issued by Ajeenkya DY Patil University, if any information furnished in the
application is found to be incorrect, if my activities are found detrimental to the interest of Ajeenkya
DY Patil University staff and other students, if found addicted to alcohol or drugs or taking part in
gambling or any other immoral act.

I hereby further undertake that if, in the event of an unforeseen situation compelling me to cancel my
admission due to my own reasons and rationale, I hereby state and undertake that the consequences
thereof such as forfeiture of fees according the prevalent rules of the university and striking off from
the rolls of the class will be on my own will and volition and Ajeenkya DY Patil University shall bear
no consequential responsibility whatsoever. If I have to cancel my admission due to my own reasons,
I confirm that I shall ask for refund only as per the prevalent rules of the university and will never ask
for a full-refund. I know that the fee once paid is not transferable under any circumstances whatever
the reason may be. In this regard, the decision taken by the management of Ajeenkya DY Patil
University will be final and binding upon me.
I am aware that Ajeenkya DY Patil University reserves the right to make additions or alterations to the
existing rules, working hours, time - table, curriculum, syllabus, mode of training as may be deemed
necessary from time to time. The students shall be required to abide by the rules that are in force and
that may be enforced in future from time to time. The university management reserves the right to
cancel, postpone the date, change / modify the venues / syllabus of any course without assigning any
reason.
I am aware that the Director of concerned school of the university under which I am studying is the
only competent authority to condone deficiency and his / her judgment and decision in all regards
shall be binding on me. I herewith submit myself to the disciplinary jurisdiction of the Director of the
School of Film and media , Ajeenkya DY Patil
University to which I am admitted.
I am aware of the provision of anti-ragging regulations and agree to submit an undertaking on Rs.
100/- non judicial stamp paper, duly signed by me and my parent/guardian and notarized as prescribed
by UGC.
I hereby sign the above undertaking willingly and voluntarily and with the full knowledge of the
contents thereof. I have retained a copy of this form for my record and perusal.
