REGISTRATION & CONSENT FORM – FREE PROGRAM/LESSON (NO FEES APPLICABLE)

PLEASE READ AND SIGN BELOW

Thank you for registering your child. Groove Nation run school dance programs in primary
schools. This free lesson is a one-off lesson. We do not run external/after hours lessons and
only operate in primary schools. These free lessons are simply for staff training and for the
continued development of our programs.

Your child will receive:

• A free lesson in a COVID-safe and encouraging learning environment

• A lesson conducted by a qualified teacher
• An inclusive and engaging learning experience that caters for all
We thank you for your support and are delighted to have your child join us.
So that everyone has a safe and enjoyable experience, we ask you to read our terms and
conditions (Terms) attached and sign below them in your capacity as parent or guardian of
the child.
1. I am the Student/ parent/ guardian of the Student named in this form (Student, you or your) and
acknowledge and agree to abide by the rules set by Groove Nation [ACN 622 578 821] (Groove Nation,
we or us) and in this form.

2. These are the Terms that apply to the contract between you and Groove Nation. Where the Student is a
minor as defined under section 21 of the Acts Interpretation Act 1987 (NSW), these Terms apply to the
Parent or Guardian of the Minor.

3. I acknowledge and agree that:

a. As the parent or legal guardian of a Minor, I give my child/ children (Child) permission to participate
in Dance Classes run by Groove Nation (Dance) and agree to abide by these terms and conditions
(Terms).
b. I am aware that my Child will participate in physical activity with vigorous activity including
movement and stretching and acknowledge and agree that I have been given the opportunity to be
examined/ have my Child examined by a licensed medical practitioner to confirm that my Child can
perform all Dance exercises and activities;
c. I will be rresponsible for any damage to the equipment used by my Child as part of the Dance Classes
or where the Dance classes are held (including the personal property of others) if it is caused due to
my Child’s action or behaviour.
d. Participation in any Dance exercises or activities could constitute a risk of serious injury to my Child
and I agree to waive any claim I may have against Groove Nation and /or the Dance instructor and
assume this risk.
e. If at any time I believe conditions to be unsafe for my Child, I will immediately discontinue their
participation in the class and seek medical assistance.

4. As the parent / guardian of my Child, I grant a waiver and release (Release), indemnify, hold harmless and
forever discharge Groove Nation, and its agents, employees, officers, directors, affiliates, successors and
assigns, coaches and teachers of and from any and all claims, demands, debts, contracts, expenses, cause
of action, lawsuits, damages, and liabilities, of every kind of nature, whether known or unknown, in law or
equity, that I ever had or may have, arising from or in any way related to my child’s participation in the
Dance class or loss or damage of personal property as a result of that Dance class, provided that this
waive of liability does not apply to any negligent act or omission by Groove Nation.

5. By providing the Release in paragraph 4, I assume, on behalf of my Child, any risk and take full
responsibility and waive and of personal injury; death, damage, or loss of personal property, associated
with the Dance classes and/or engaging in any related event or other related activities on and off the
premises held by Groove Nation.

6. Groove Nation reserves the right to cancel any planned event or class, with no guarantee of refund and
any rescheduling of such event at its sole discretion. However, we will endeavour to ensure any event
cancelled because of our illness or other issue is re-scheduled. Where an event is cancelled due to
circumstances out of our control, we reserve our rights to cancel without re-scheduling any such event.

7. We reserve the right to cancel your enrolment with Groove Nation where any of the following occurs:

a. if you or your Child fails to comply with any reasonable direction issued by any staff member
of Groove Nation which compromises our ability to provide our services to students or
compromises the safety and welfare of other students of Groove Nation;
b. for any other reason outside our control which has the effect of compromising our ability to
perform the services within the required timeframe;
c. if, in our sole discretion, we consider that it is no longer appropriate to provide classes to you.
d. you may cancel your enrolment by written notice at any time, noting that the cancellation
will take effect on the last day of the term you are currently enrolled or at the end of the
 term in which date of cancellation has been notified,

8. You acknowledge and agree that dance classes may be provided online or in person, at our discretion.

9. Groove Nation provides the services and dance classes on the assumption that your Child is fit to
participate. By accepting this agreement, you acknowledge and agree that your Child is fit to participate in
the class.

10. No food is permitted to be consumed during a class or event or permitted into the dance area at any time,
including where you attend online classes. Students are required to bring water to hydrate themselves
and have that water to hand whether the class is an in person class or online class..

11. Groove Nation is committed to providing an enjoyable experience for all Students. Groove Nation will not
tolerate the following forms of unacceptable behaviour, whether at in person classes or events or during
online classes and events:

a. verbal or physical abuse;

b. intimidation by a student or another student or intimidation by a Parent or Guardian of any
student (including the Child of the Parent of Guardian);
c. discrimination in all its forms as defined under the Anti-Discrimination Act 1977(NSW) and
other equivalent state and territory legislation around Australia and globally; and
d. harassment or threats of any nature towards Student, Parents and Guardians, or any
employees or sub-contractors or third parties attending the Academy.

12. No photograph and video recordings shall be permitted of any classes or student performances, wherever
they may occur. Where a Student, parent or legal guardian fails to comply with clause, we reserve the
right to cancel the enrolment of the Child of that parent or legal guardian at our discretion.

13. We may choose to communicate you with email or other forms of electronic communication (Email).
However, as this type of communication may not be secure and it may be copied, recorded, read or
interfered with by third parties while in transit, if we send you Email, you release us from any claim you
may have as a result of any unauthorised copying, recording, reading or interference with that document
after transmission, for any delay or non-delivery of any document and for any damage caused to your
system or any files by a transfer.

14. Due to privacy and child protection obligations under State and Commonwealth legislation, no parent or
adult shall be permitted to enter any dressing room, or area/space that may be used for children to
prepare or finish classes or prepare for an event. Where a parent or legal guardian fails to comply with
this clause, the enrolment of their Child will be cancelled and the Child and their parent or legal guardian
will not be permitted to attend any premises or performances of Groove Nation.

15. These Terms contain the entire agreement between parties, and supersedes any prior written or oral
agreements between them and the provision of these Terms will continue in full force and effect even
after the termination of the Dance classes conducted by, on the premises of Groove Nation, whether by
agreement, by operation of law, or otherwise.

I understand and confirm that by signing this document I agree to be bound by these terms and have given up any
right to claim against Groove Nation.

Optional Social media release (you MUST advise us if you DO NOT want your child photographed/recorded)

My Child may be photographed while taking Groove Nation Dance classes. These photographs are the property
of Groove Nation and may be used to promote Groove Nation on social media and in other marketing material.
I acknowledge that no compensation will be provided for the use of these photographs.
