Student Agreement

This student agreement is designed to protect all students of City University of Hong Kong (the University)
participating in activities off campus. The Global Engagement Office (GEO) requires all students to sign
this form to indicate agreement before participating in off-campus activities organized by GEO. All students
must sign and return this agreement to GEO prior to the activity.

1. I acknowledge that I participate in the activity voluntarily.

2. I have carefully considered my health and personal circumstances when applying for/agreeing to
join this activity, and I am physically and mentally capable to participate in and complete the
activity; and I agree to assume all responsibilities for my health, safety, and belongings while
participating in this activity.

3. I release the University, its officers, employees, agents and contractors from any and all liability,
damage or claim of any nature whatsoever arising out of, or in any way related to my participation
in this activity; to the extent as permitted by the applicable laws.

4. I agree to indemnify the University from any damage or liability incurred by me:
(a) as a result of any illness I may suffer; or
(b) as a result of my act or omission
including the costs of any medical care, or any injury or damage to the person or property of others
which I may cause, or from any financial liability or obligation which I may personally incur, while
participating in the activity.

5. I understand that GEO reserves the right to make cancellations, changes or substitutions regarding
the activity in case of emergency or changed conditions or in the interest of students concerned
including myself. Should GEO cancel the activity, refunds may be made according to the
University’s standing policies and procedures, or any other arrangements that GEO may consider
appropriate. I accept that I do not have any claim against GEO, its officers, employees, agents and
contractors regarding such cancellations, changes or substitutions.
6. I understand that all full time students (including exchange students) are covered by the Group
Personal Accident Insurance Policy arranged by the University. Notwithstanding, I understand and
agree that I will be financially responsible for expenses that are not covered by the above-mentioned.

7. I understand that it is my responsibility, to understand and comply with the terms of participation,
codes of conduct, rules and regulations of the activity, and laws and customs of Hong Kong. The
University is not responsible for the defense of a student accused of a violation of the laws and is
not responsible for the payment of any fines or other penalties resulting from such violations.

8. I understand and agree that no alcohol is allowed throughout the OCamp.

 9. Hereunder, I provide at least one emergency contact number of my parent(s), guardian or next of
kin to the organizer of the activity. By providing this emergency contact number, I agree to
authorize the organizer of the activity to release my personal information (as the case may be) to
third parties external to the University on a “need-to-know” basis should emergency situations as
determined by GEO occur. This authorization shall continue to be effective until the end of the
activity.
