Lead School Transition Service is an advice and guidance service for

the eight Christchurch fundholding secondary schools:

Allenvale, Cashmere, Ferndale, Hillmorton, Mairehau, Papanui,

Riccarton, Waitaha

The service will target students who are identified as having a

disability (ORRS and non-ORRS)

The service is a project that supports schools to improve the

pathway to post school life.

The Lead School Transition Service is underpinned by the Wayne

Francis Charitable Trust / Creative Solutions ten point best practice
guidelines.

The Lead School Transition Service will provide:

• Information
• Support for developing partnerships between schools
and services
• Transition planning processes and templates
• Professional Development
• Support and Advice
• Critical intervention for identified students and their
family/whanau
