Dear Parents or Guardians,

The fourth grade class has two field trips in October and one in May this school
year. Every year we have an overwhelming response to our request for chaperones. We
would like to find out who would be interested in chaperoning one of our field trips. We
are allowed 4 chaperones per trip. Please return this form with your choices circled( 1st,
2nd, 3rd). For the October trips we will let you know by Sept. 23rd

El Centro Math and Science Day at Colorado State University

Denver Capitol Tour and Colorado History Museum--

Greeley Centennial Village History Festival-- full day on

Will be announced in the spring. Cost $3.00 per person

Please fill out any of these forms if you would like to join us on one of our
exciting field trips. Please return by Sept. 18th. We will confirm and send you the
information for the event you will be chaperoning a few days prior to going on our trip.
Please send no money. We will collect this after you have been contacted for a specific
trip. No younger or older siblings will be allowed to attend field trips. These are
fourth grade PSD student activities only.
