„ มีความแปรปรวนของ
- ความบอยของการถายอุจจาระ „ Un-investigate dyspepsia
- ความแข็งเหลวของอุจจาะ อาจเปน Non-ulcer หรือ Ulcer dyspepsia หรือ
- มีความรูสึกในขณะถาย เชนถายไมสุด ถายเบง Cancer
„ อุจจาระมีมูกปน

Initial management
„ Exclude life-threatening condition
„ Symptomatic treatment
„ Observe

สัญญาณเดือน (Alarm features)
Dyspepsia
R/O typical biliary pain, IBS 1. Age of onset > 40 years
-ve +ve 2. Awakening pain
สัญญาณเดือน (Alarm features) 3. Significant weight loss (> 5% BW within 1 month or > 10% within 3 months)
4. History of GI bleeding
5. Persistent vomiting
ใหสุขศึกษา เปลี่ยนแปลงพฤติกรรม พิจารณาสงตรวจ 6. Dysphagia
+ รักษาตามอาการ ที่ไม invasive 7. Strong family history of GI malignancy
Antisecretory and/or prokinetic for 2-4 weeks 8. Anemia
9. Jaundice
10. Hepatomegaly, splenomegaly, lymphadenopathy
พบความผิดปกติ 11. Fever
อาการดีขึ้น
ใหยาตอ 6-8 สัปดาหื ไมดีขึ้น 12. Abdominal mass

ยาที่ชวยลดอาการไดอยางมีนัยสําคัญทางสถิติ เมื่อเทียบกับยาหลอก
„ PPI 1 H2 antagonists (Ranitidine)

„ Prokinetic druge ยาที่ชวยลดอาการมากกวาเมื่อเทียบกับยาหลอก แตไมมีนัยสําคัญทางสถิติ

1 Proton pump inhibitors ( RRR, 12 % 95%CI, -1 to 24)
2 Bismuth salts

ยาที่ไมชวยลดอาการมากกวายาหลอก
1 Antacids (RRR, -2 95%CI, -36 to 24 )
2 sucralfate.

*Prokinetic agents ( Metoclopamide, Case sceinario

Domperidone) อาจมีประโยชน โดยเฉพาะในรายที่อาการอืดเดน
*Antispasmodic ( Buscopan) อาจไดประโยชนในการบรรเทา
อาการระยะสั้น
#Simethicone (Air-X) อาจมีประโยชนแตตองรวบรวมหลักฐาน
