AMERICAN HIGH SCHOOL 2014 PROM GUEST APPROVAL FORM

This form must be completely filled out and turned in when you purchase your Prom Bids
NOTE: A copy of the guests student ID (in a readable form) must be photocopied and stapled to this form. If the guest is a college student or IS NOT in school at all, you must attach a copy of their driver license to this form (May not be over 21) They must also bring their ID to the dance! Students must be dressed in proper attire. Guests must be accompanied by their AHS host at the door. BE AWARE THAT STUDENTS WILL BE SEARCHED AND THE ALCOHOL BREATH ANALYZER MAY BE IN EFFECT prior to the dance. Students must stay at dance until 11pm, and once they leave they cannot return and are then the responsibility of their parents.
