I would like to know more about you and your child's family life, as it is very important to me that the student's family and culture are found in the classroom. Please take the time to fill out this family questionnaire so that I may know more about you and your child!

We love to have families volunteer in the classroom and it is important to be involved in your child's school life! I would be interested in having you be involved in any way possible. What days of the week would you be available, and how much of a time would you like to commit to volunteering in the classroom? If not available during school hours, would you like to help out in other ways? (i.e. evening events, take home things to cut out, etc.) Are there any other family members/relatives that would want to volunteer their time?
