Requirements for Scholarship Consideration

High school senior, who is graduating in 2014

Recipient must maintain a minimum 2.5 GPA
High school transcript
College or University acceptance letter or Class Schedule
A complete and signed Scholarship application
Recipients must attend the scholarship event on Sunday June 1st, 2014 6:00 8:00 PM. The
location will be announced at a later date.
Recipient selection is based on scholastic achievement and community involvement.

Scholarship Eligibility Checklist

I have completed this application in full.

I have attached my high school transcript reflecting my current GPA and grade level.
I have attached an acceptance or verification letter from the college or university of my choice
Disclosure
I understand this application becomes the property of the American Moslem Society (AMS). I authorize
AMS to obtain any additional information needed from the school. Scholarship recipients will be chosen by
a committee chosen by the AMS. All decisions made by the committee are final.
