Job Description:

Actual Work Performed:


1. Advocate for clients and their families during the reentry process from the hospital into the

Job Description:
Responsible for rendering nursing care in a defined range of Health care services,
assessment, implementation, and evaluation as well as teaching and related services such as
counseling. This includes as follows: provides direct and indirect patient care services, performs and
deliver basic health services; assess signs and symptoms of illness; administer medications per
physicians order; observe medication responses and or side effects; observes general physician and

Actual Work Performed:

1. Prepares the bed of the patient, see to it that clean and exclusive ICU
linen and gowns are available.
2. Prepares the Medical equipment as needed.
3. Receives verbal report from E.R., O.R., Ward nurses and should be
described:
Age of the patient and general condition.
Medical diagnosis, pathology.
Fluids administered, blood loss and replacement, tubing and drain present.
Specific individual problems or deficits, hearing vision and mental status.

4. Receive patient from E.R., O.R., and Nursing area.


Provides immediate care like:
Positioning of patients according to needs.
Monitoring vital signs.
Hook patient to the monitor as ordered by the physician.
Provide well-ventilated room, change patient clothes to IC gown.
5. Report losses, breakages and malfunctioning equipment to charge nurse.
6. Maintain the cleanliness of ICU as a whole.
7. Recognize and report any untoward manifestation of patient. RE: changes in ECG tracing,
significant vital signs changes, patient change in sensorium, etc., anything that is life
threatening to the patient must be reported immediately to the attending physician.
8. Perform the nurses independent functions.
9. Endorse patient status clearly and completely to ward nurses.
10.Attend continuing education program to be able to improve ones skill and learn new updates
in nursing.
