Nurse Practitioner

JOB DESCRIPTION Nurse Practitioner needed for the Family Medicine department. Full
time, four days a week, on- call requirement, and weekends off. The NP provides patient care;
conducts physical exams; completes health history summary; documents patients' progress and
assists with patient care management.
JOB SUMMARY
Nurse Practitioner will provide primary care services for certain acute or chronic health
conditions in specific populations in accordance with applicable scope of practice and with the
policies, values, and the mission of the organization.
JOB RESPONSIBILITIES
1. Obtains and reviews patient's medical history information relevant to patient care.
2. Orders and interprets diagnostics tests and screening evaluations.

Perform appropriate office procedures that are within APRN scope of practice.
Prescribe appropriate pharmacological and non-pharmacological treatment modalities.
Collaborate with multidisciplinary team members by making appropriate referrals.
Educating patients/families and their communities about disease prevention and healthy
lifestyle habits.
7. Managing overall patients care regarding health issues.
8. Conducts follow up calls to patients to discuss the patient's progress and address any
concerns.
REQUIREMENTS

Graduate of an accredited Nurse Practitioner program

Valid Advanced Practice Registered Nurse (APRN) licensure and DEA
Current BLS and ACLS
Excellent communication skills
Strong computer skills
Ability to build positive relationships with patients, families, providers and staff.
