For employees of Cape Fear Valley Hospice

To

help patients and families


who are in need to pay utilities
or provide food, transportation
costs, toiletries, or any other
need which will be approved
by the agency

Each bi-monthly pay period, $2.50


will be deducted from participating
employees wages (for a total of
$5.00 per month) and will be paid
into a fund that will be managed by
the office manager
This is a totally voluntary program
for participation by employees

Once

the fund reaches $1,000.00,


funds will be available for use
Patients and families can be
nominated for assistance by any
employee who is made aware of a
specific need
Social worker will administer the
program and approve the request

Employee

will complete an
application and submit to social
worker
Social worker will verify need and
then request check for payment
from office manager

One-time

assistance in an amount
not to exceed $100.00
Assistance includes but is not
limited to the following: utility
bills, food, toiletries, gas for
appointments

Monthly

drawing for Employee of


the Month parking spot
Ability to wear jeans on the last
Friday of the month
Gold star for employee badge
