Guardianship Life Book

Guardianship
County takes responsibility for the persons
welfare.
Typically a person with serious physical and/or
mental disabilities
If placed outside County, they are still the
Countys responsibility
Once placed in adult care home, social worker
assigned to case regularly visits and monitors
the health and well-being of the client.

Guardianship Life Book


This book contains several important
documents pertaining to each ward:
Usually a recent picture
Important information about their
background, family (if any) and placement
Court order granting guardianship of person
to the county.
Annual Status Reports, which are required by
the court.

Purpose of the Agency Project


The current Life Book at the agency was not
completely updated
Several wards were missing pictures or vital
information
The Life Book was contained in a single
large binder which was bulky, messy and
made it difficult to find information on
individual wards.

Actions Taken
Social workers for each ward was asked to
get a new photo.
Information was updated.
The Life Book was reorganized into two
separate binders. One with photos and vital
information, another with legal documents.
Both sections were organized and tabbed
with a clear table of contents.
