JUDICIAL AFFIDAVIT
I,
EMMANUEL PACQUIAO, of legal age, Filipino,
and a resident of Burgos St., Tuguegaro, City after having
been duly sworn to in accordance with law depose and say:
The person examining me is ATTY. MA. DIANA S.
VELASCO. The examination is being held at
Tuguegarao City. I am answering his questions fully
conscious that I do so under oath, and may face
criminal liability for Perjury or False Testimony.
Q1: Sir you mentioned that your name is EMMANUEL
PACQUIAO, do you have any relation to the petitioner in this
case?
A1: I am the petitioner who filed the petition for correction of
entries in my certificate of live birth.

Q3: Do you have any proof that your name is EMMANUEL

PACQUIAO?
A3: Yes, Sir, I have with me my Certificate of Baptism and
Voters ID.
I am marking the Certificate of Baptism and Voters ID
as Exhibit A and B respectively
Q4: Why did you file this petition?
A4: Because I wanted to change my name to MANNY
PACQUIAO because it is the one popularly known to the
public, all my school records, employment records and other
documents.
A9: Pursuant to the Order of the Honorable Court dated
December 12, 2014, I served a copy of the petition to the
Office of the Solicitor General and caused the publication of
the said Order for three (3) consecutive weeks.
Q10: Do you have a copy of the said Order of the Honorable
Court?
A10: Yes, attorney. I have it with me.
Q: Do you confirm and affirm the matters in this judicial
affidavit?
A: Yes, Attorney.

I, ATTY. MA. DIANA S. VELASCO, of legal age, Filipino

and a resident of Tuguegarao City, attest under oath that:
I personally conducted the interrogation of EMMANUEL
PACQUIAO, the petitioner in the Petition for the Correction
of Entries in the Certificate of Live Birth, and I faithfully
recorded the questions I asked and the corresponding
answers he gave me, and neither I nor any other person
present coached him regarding his answers.
