List of Problems

1. Ineffective tissue perfusion related to decrease in hemoglobin secondary to anemia.

Prioritized problem

Justification

Ineffective tissue perfusion related to decrease

in hemoglobin secondary to anemia.

This is the first prioritized problem as it relates

to the patients circulation and according to the
ABCs of prioritization, circulation is a top
priority. This is our first problem because over
all the listed problems, this diagnosis is the
one that may have the greatest negative effect
on our patient if not tackled. By prioritizing
ineffective tissue perfusion first and making
swift solutions for the complications that follow
it, we could prevent further problems from
occurring with our patient.

2. Risk for infection related to Inadequate

This diagnosis is also related to the circulation

but because it is only a potential problem, it is
second to Ineffective Tissue Perfusion.
Although it cant be helped that the chemo
medications make our patient
immunocompromised, the risk for infection is
still second in prioritization because we need
to make sure that our client isnt exposed to
more deteriorating factors

secondary defenses and immunosuppression

secondary to side effects of chemotherapeutic

drugs.

3.1 anticipatory grieving related to impending

death

This is our third problem because it does not

fall within the three categories of the ABCs of
prioritization. Although anxiety can be an issue

3.2 Anxiety related to impending death

a. Imbalanced nutrition:less than body

requirements related to consequence
to chemo therapy and emotional
distress

b. Disturbed body image related to

c. Hopelessness related to

for the patient. It is only third to Ineffective

Tissue Perfusion and Risk for Infection
because its least likely to cause any physical
harm as quickly as the top two prioritized
problems.

Underneath Anxiety, imbalanced nutrition is a

top problem that can be caused by the feeling
of anxiety. Due to this anxiety , the patient
feels that even if she will eat still she will be
dead soon.

Also under anxiety, disturbed body image may

fall here as the second sub-problem the
change in body image can contribute as a
factor that instigates the feelings of anxiety of
impending death.

This problem, which inevitably occurs if anxiety

is not controlled, can exacerbate to feeling of
being unable to move on which would affect
the clients quality of life.
