7:00-3:00 pm F> Disturbed thought process related to severe anxiety or depressed


mood as evidenced by memory problems and impaired abiltity to
grasp ideas or orders thoughts
D> Received lying on bed; “ Hindi ko na alam kung anong uunahin
kong ayusin, gulung gulo na ako.”; with reported life stress,
depressed and being anxious; restlessness noted; with constant
episodes of crying spells; irritable; with disordered thought
sequencing noted
A> Assessed attention span/ distractibility and ability to make
decisions or problem solve to determine ability to participate in
planning and executing care; assessed ability to receive, send and
appropriately interpret communications to assess degree of
impairment; determined previous level of cognitive functioning from
his family, or past medical records to evaluate for his progress;
provided safety measures such as side rails, padding as necessary and
close supervision as indicated; maintained a pleasant, quiet
environment and approach him in a slow, calm manner; minimized
words which she cannot comprehend instead use simple and concrete
words as to not impair comprehension; minimized his responsibility
especially when severely depressed to decrease feelings of guilt,
anxiety and pressure; maintained adequate rest periods to provide
stimulation while reducing fatigue; performed periodic neurologic
assessment as indicated; maintained reality oriented relationship and
environment; encouraged to verbalize true feelings to help in
resolving issues; educated her to intervene using thought stopping
techniques, when irrational or negative thoughts prevail; encouraged
her to reduce provocative stimuli, negative criticisms, arguments to
avoid triggering fight and fight responses; educated her to reframe or
refute negative thoughts to reduce depression
R> The patient was able to show improved mood and exhibits
organized thought process
