Introduction  The patient’s name is X, 47
years old, male and from
 My name is Liza MedWard renal.
Puroc, 23 years
old from BCU. I
am calling
because of the
case of my
patient who is
suffering from
ESRD. 

S
SITUATION
 The patient’s name is X, 47
years old, male, with current
stable situation but in danger
of deterioration. With
noticeable weight loss,
inability to ambulate and
observed to have peripheral
neuropathy. Closely monitor
for VS especially BP and I&O
and daily weight. With high
lab results of BUN, GFR and
creatinine.

B BACKGROUND
 The patient’s diagnosis is end
stage renal disease with a
history of type 2 diabetes
mellitus with a complaint of
weight loss, fatigue and
weakness. Currently having
an IVF of PNSS to run for KVO

A ASSESSMENT &
ACTION
 My assessment on the patient
is that he has a noticeable
weight loss, weakness,
nauseated, frequent vomiting,
anorexic, and is hypotensive.
The patient’s need is to be
closely monitored for I&O,
daily weights and VS.

R RESPONSE, RATIONALE
& Recommendation
 My recommendation is that he
should maintain his body
weight in normal range to at
least have enough energy to
support his body to prevent
further complications. I also
recommend to always have
someone beside the patient to
watch out for any possible
complication and to assist the
patient whenever he has
difficulty in doing his self care
activity.
