Classroom Behavior Plan

Dear Parents/Guardians,
Below are the rules that are a staple of Mr. Stotts 7th grade Math Classroom. The five rules
which are listed below are those that will dictate the students behavior in the classroom. The five
rules are: Be prompt, Be prepared, Be positive, Be productive, Be polite. These rules are in
place in order to create a warm, welcoming environment conducive for learning. If any one of
the rules is not followed, the consequences below will be enforced.
Following two verbal warnings, the student will be required to fill out the Warrior Behavior
Reflection form. This form, which is attached, is a way for the students to outline what happened
prior to the incident/ what occurred to lead to the undesirable behavior, the actual undesirable
behavior and then allow the students to provide consequences that they feel are fair for their
behavior. Once finished filling out the top portion the student will have to propose a plan to
correct their behavior and how they will limit the undesirable behavior in the future. This form
will be a mediation tool between two students when getting into an altercation. This will allow
them to share their side of the story, the altercation and the consequences then find a more
appropriate way to handle the situation.
Consequences:
First Offense: Verbal Warning
Second Offense: Verbal Warning
Third Offense: Fill out Warrior Behavior Reflection Form
Fourth Offense: Contact Parents to set up a parent/teacher conference

Behavior Reflection form and support the use of it in the classroom.
