As part of our outer space theme, we will be taking a field trip to Griffith Observatorys Planetarium.
Please read the information at the top of this form and detach, sign, and return the bottom half to your
childs teacher by March 2, 2015. Thank you!

Cost: $15 (includes admission, lunch at the End of the Universe Cafe, and transportation)

Time Returning to School: 1:00 PM

Special Instructions: Please provide a light sweater that can be tied around your childs waist. Be sure to
write your childs name on the tag of their sweater.
If you will be available to chaperone for this trip, please indicate so on the bottom of this form. Our
kindergarten class has 20 students, we are asking for 5 chaperones. Our first grade class has 25 students,
we are asking for 5 chaperones as well.
