Dear Mr. & Mrs. Smith:
I am writing to you in regards to Susie. In recent weeks I have noticed a slow decline in
Susies participation and quality of work in science class. Compared to last semester,
Susie has been much less organized and much more off-task during in-class work. In
addition, Susie seems to have a much harder time recently meeting deadlines for
homework and in-class projects.
The purpose of my letter to you today is first to bring to your attention my concerns over
Susies off-character behavior. She had always been so bubbly and involved in class.
However, her recent behavioral changes have me somewhat concerned and I hope to
bring that to your attention. On those occasions that I have talked to her, she has
expressed how very tired she has been.
I believe that if we work together, we can get Susie back on track for the semester. I
believe that at least two hours of tutoring per week would go a long way on catching up
on missing assignments. I can be available for Susie one hour, twice per week on
mutually agreeable days. In addition, I believe that if Susie can spend 30 minutes per
night reading through her notes, she would be prepared to retake her Unit 3 exam this
coming Friday.
Thank you for taking the time to address these concerns with Susie and myself. Please
contact me with days and times that would best suit Susie for tutoring. I also look
forward to any suggestions or concerns you might have in regards to Susies academic
success this semester.
