We are asking your office to enroll MR. CYTON MENDOZA, in the following subjects of approved in
his study load attached to this letter. The attached study load form, signed and approved by me an as
evaluator, was accomplished by his mother as well as all the enrollment process and paid necessary
payments. Yet, as to the input of his subjects, his mother failed to inform him that he should be the one to
enroll it online. Therefore, currently he is not enrolled in his desired subjects.
Mr. Cyton Mendoza is one of our students in the International Studies program who went to the US for his
OJT under the partner agency HRI. He is a graduating student, and down to his last 3 subjects to qualify to
graduate in the program. But if failure to accomplish these subjects, he will not graduate this semester.
It has been admitted fully and base on thorough investigation, his mother is at fault in why they had a
miscommunication regarding his enrollment status. Mr. Mendoza recently arrived and wished to enter his
classes yet as he looked at his SLMIS, he is currently not enrolled in any of the subjects approved to him to
qualify.
With this, we would like to ask your good office to accommodate the subjects. I hope for your most
favorable response and as always, I thank you for your generosity and support of our endeavors.
