SAC Club and Association Audit Information Form AU

All clubs/associations receiving SAC funding must submit complete information for auditing by the indicated deadline. Failure to
do so or incomplete information may result in the termination of your SAC funding.

Please provide a detailed outline of your expenses and how they have benefited the student body, and attach all receipts,
invoices, etc to prove necessity of cost.

The following form is designed to assist, but it is not required. Please submit your information to the SAC Mailbox or online
