I began my visit with the couple by visiting with the wife. As we
were discussing her medications, she suddenly became acutely
confused. She was staring at the bottles, not knowing anything
about the medications and said, I feel dizzy; something is not
right.
I did a complete neurological assessment and did not find
anything out of the ordinary. In reviewing her history with her
husband, he mentioned she had previously had a transient
ischemic attack (mini stroke).
I conveyed to them that the sudden confusion could be another
stroke and urged a trip to the emergency room for evaluation
which she refused. So, we compromised and I called her primary
care physician who instructed her to go to the emergency room.
The ER notified me when she arrived and I discussed my
observations with the doctor there. They appreciated the findings
and said they were admitting the member for observation after
obtaining CT scans of her head. As it turns out, the CT scans were
negative and the origins of her sudden confusion were unknown.
But the husband said they were very appreciative of my visit and
looked forward to seeing me next time.
