    Email from EPA (Aaron Setran) to PTP (counsultant Brenda Stein re: how to fill out UIC permit application. Sent on 3/31/09 at 9:34 a.m. Mr. Setran wrote, "Since PTP is applying for re-issuance of an existing permit, the attachments are not required. Just have PTP fill-out the form, reference the existing permit, and send it in w/out attachments. We already have most of the necessary info on file.     Permit renewal questions

    What should we use for the SIC code?


    Some of the attachment questions/requirements I simply do not have adequate information for a proper reply.
    A. Area of review methods I think this is already set. Is there anything I would need to do now for
    this?
    B. Are the plans that we have completed for the entire system adequate. If Leon purchases the flight
    info, we could have that topo. I am thinking I need to ask him to purchase the information.
    C. Are the reports we have submitted good for this. For the corrective action plan, do we just need to
    point out that we can turn all the fields on and off as needed?
    D. Does this information already exist?

    F. Does this information already exist?

    H. Can I just attach the proposed permit, or the existing permit?
