Step 1
Purchase a new qualified Polycom conference phone between August 15 and September 30,
2011. (See table of qualifying products)
Step 2
Submit completed Conference Phone Rebate Claim Form, proof of purchase, and customer
invoice or receipt via mail, fax or e-mail as directed on the claim form. All claim forms must be
submitted within 30 days of purchase.
Step 3
Polycom will issue a rebate check directly to the end user customer within 6 weeks of
receiving qualified and verified claims.

Terms and Conditions
1. End users must purchase a new qualified Polycom voice conferencing unit from a Polycom channel partner.
2. Polycom must receive the claim form with all required documentation within 30 days of purchase. Note: If the claim form and
documentation are not received within 30 days from date of purchase, program participation will be declared incomplete, and the end
user customer will not receive the rebate.
3. Required documentation includes: a. Completed claim form
a. Completed claim form
b. Proof of purchase (white label with serial number from product box)
c. Customer invoice or receipt.
4. Proof of purchase listed in #3 above must be the cut-out white label from outside of the box that includes the serial number. Mail-in
submissions must include the original proof of purchase. Fax and e-mail submissions may include a photocopy or scanned image of proof
of purchase.
5. The end user will be sent an e-mail confirmation when their claim is received. Polycom is not responsible for lost or misdirected
shipments, e-mail, mail, or faxes.
6. Polycom has the right to call the end user at any time for verification purposes.
7. This offer is only available to end user customers who purchase from a Polycom channel partner.
8.This offer is void where prohibited or otherwise restricted by law. Rebates will be sent within 6 weeks of receiving all the necessary
documentation (completed claim form, proof of purchase, and receipt or invoice).
9. Program is valid from August 15, 2011 through September 30, 2011 in North America or until terminated by Polycom.
10. This offer may not be combined with any other promotional offer, trade-in program or specially-priced multi-packs. Polycom reserves the
right to modify or cancel this program at any time.
