& "Whatta Quack" Order Form
In addition to the Duckie Derby race, Decorated Duck Contest, Food & Fun on
Saturday, May 22, 2010.
Duckie Derby Day opens at 1:00 pm.
We are offering two new opportunities to support St. Catherine Academy

1) For a $50 donation you can "Duck, Duck… Goose!" a friend or family member. We will mail
them a friendly "You've been Goosed" certificate and a sponsor race duck number!

2) For a $250 donation you can "Whatta Quack" a friend. Our Duck Mascot will visit the home
or office of a friend in Fairfield County with a friendly "You've been Goosed" certificate, a
sponsor race duck number and a Decorator Duck! Whatta QUACK!

Enclosed is my check made payable to St. Catherine Academy

$50 check, please mail the "You've been Goosed" certificate and a sponsor Race Duck
Number to the person listed below.

$250 check, please have the duck mascot "Whatta Quack" visit the person listed below,
present the "You've Been Goosed" certificate, a sponsor race duck number and a Decorator
Duck to them. *We will contact you to plan/schedule our visit to your friend below.
