AFFIDAVIT OF REQUEST FOR CANCELLATION OF HOLD DEPARTURE ORDER

NUMBERS RMA-09-282, ADD-2001-535,ADD-2001-588

I, ISAGANI SYBICO AMATONG, Filipino, of legal age, and a resident of Liloy,

Zamboanga del Norte, after having been duly sworn in accordance with law, do hereby
depose and state:
1) That sometime in the year 2001, two (2) Hold Departure Orders have been
issued by the Bureau of Immigration, (ADD-2001-535 and ADD-2001-588),
against the affiant in relation to a case filed against him by former Congressman
Llorente, in his capacity as then Governor of the Province of Zamboanga del
Norte, (Annexes A, B);
2) That on May 09, 2009, another Hold Departure Order, bearing the number RMA09-282 was issued for another case filed against the affiant relating to the
exercise of his function when I was Governor of Zamboanga del Norte, (Annex
C);
3) That based on DOJ Department Circular No.41 which gives any HDO issued by
the Department a life span of five (5) years from the date of issuance unless
sooner terminated, HDO Numbers ADD-2001-535, ADD-2001-588 and RMA-09282 issued against the Affiant have already expired;
4) That aside from the fact that RMA-09-282 has already expired, the case in
relation to which it was issued, has been dismissed with finality by the
Sandiganbayan last September 18, 2013, (Annex D);
5) That based on the foregoing reasons, the Affiant requests for the cancellation of
the said Hold Departure Orders, upon full payment of legal fees related therein;
6) That, furthermore, the Affiant respectfully requests the speedy cancellation of the
mentioned HDOs in order for him to fully exercise his functions as incumbent
Representative of the 3rd District of Zamboanga del Norte which requires him to
travel to Southeast Asian nations this January, in preparation for ASEAN
Integration and Consultative Meetings for the proposed enactment of
Bangsamoro Basic Law;
7) That I hereby authorize Maia Chiara Halmen Reina A. Valdez, my Congressional
Staff and Lawyer to facilitate the cancellation of the abovementioned HDOs and
payment of corresponding fees;
8) That this affidavit is being executed for presentation to Immigration authorities as
part of the requirements for the cancellation of Hold Departure Orders.
