AFFIDAVIT OF DISCREPANCY
I, LERMA S. TUMAMBING, of legal age, Filipino, widow and a resident of
Blck. 29, Lot 15 Mercedes Home, San Miguel, Sto. Tomas, Batangas, after having
been duly sworn to in accordance with law, depose and state:
1. That I am the wife of WILFREDO P. TUMAMBING, born on April 16, 1955 in Sto.
Tomas, Batangas;
2. That when I secured a copy of his birth certificate from the Local Civil Registrar of
Sto. Tomas, Batangas, a discrepancy with respect to his birth date appeared therein;
3. That, instead of the true and correct birth date April 16,1955, it erroneously
appeared therein as August 15, 1955;
4. That, in truth and in fact, his true and correct birth date is April 16, 1955 and not
as August 15, 1955;
5. Likewise, when I secured copy of our marriage contract from same Office, a
discrepancy with respect to his mothers name appeared therein;
6. That, instead his mothers true and correct name Julia Pabilin, it erroneously
appeared therein as Julia Pagilin;
7. That, in truth and in fact, his mothers true and correct name is Julia Pabilin and
not Julia Pagilin;
8. That I am executing this affidavit to attest to the truth of the foregoing facts for
support in claiming the benefits of my husband, Wilfredo P. Tumambing and for
other legal intents and purposes it may serve.
