Filipino citizen and a resident of Sagpon, Daraga
Albay after having been duly sworn to in
accordance with law, hereby depose and state

2.That I am applying/ availing CHED Scholarship
3.That I have never enjoyed nor applied for any
scholarship grant:
4.That I am executing this affidavit to attest to
the truthfulness of the foregoing facts and for
whatever legal purpose this may serve.
