Review of System

Psychological:
Patient L.L is coherent and conscious during the shift. According to the patients
relative patient L.L. is a happy-go lucky person. He just spent his money to alcohol and
friends to have fun. He will go home drunk at night because of the same reason that his
daughters didnt have time to visit him. He gets irritated easily to anything he doesnt
like. Patient L.L. is living together with his other relatives and when it comes to financial
they dont have a problem because her daughters give him an extra allowance for his
others necessities (medication, for his dialysis, food and etc.). Every Sunday, her niece
inviting her to go to church but he always says that Isama mo nalang akong sa dasal
mo. Patient L.L cannot hear any single word at a far distance but he is responding with
a nod when you talk loud and clearly near to him. He always utter uwi,uwi because he
wanted to go home and to get discharge-out in this hospital
Elimination:
Patient L.L. is able to urinate without discomfort before, but now he has having a
hard time when urinating and a diaper was applied to him.
Rest and Activity:
He cannot get a good sleep after he was admitted to this hospital. Before, every
morning he go for a walk for about 15 minute but he is now he cannot stand nor sit on
his own.
Safe Environment:
The patient doesnt have any allergies in foods and medications.
Oxygenation
He used oxygen cannula to breathe properly. Respiratory rate of 36 cpm
Nutrition
According to patients relative patient L.L. doesnt want to eat solid foods and he
doesnt drink water instead he drinks flavored juice and softdrink. He only drinks milk
and eat congee (lugaw) before. His food and medicines are administered via NGT.
