RELEASE OF LIABILITY: In consideration of ACTIVE SPORTS ACADEMY permitting my children/myself to take part in the ACTIVE SPORTS ACADEMY programs. I hereby
agree to waive any and all claims of any kind whatsoever howsoever caused by any of the releasers or not. This is to certify that I am the parent/ legal guardian/adult of
the above and also agree to the terms of the Release of Liability as stated above and therefore have no objection to my son/ daughter/myself taking part in the
coaching programs.
REFUNDS: I understand that once the above student has attended the first class, payment is due in full for the course and the student will not receive a refund or
transfer of fees for any missed classes should the student be absent or withdraw from the classes for any reason.
I understand PHOTOS are sometimes taken and agree that they may be used for future promotional and marketing purposes.
