AFFIDAVIT OF MODUS OPERANDI

I, Rev. Fr. Dale Del Rosario Duallo, of legal age, single, with address at Poblacion,
Inabanga, Bohol after being sworn according to law, hereby depose and say:
1. That I am the President of St. Paul the Apostle Parish, Inabanga Bohol, Inc. , a non-stock,
non-profit organization incorporated at Poblacion, Inabanga, Bohol.
2. That the said organization shall implement the following projects and activities in
the next two (2) years:

4. That on October 15, 2013, a 7.2 magnitude earthquake hit Bohol which caused
destruction of St. Paul the Apostle Parish Catholic Church (the Church) with only the
faade and the back remained standing. Consequently, a group of parishioners headed by
the Parish Priest organized the Company with the primary purpose of reconstructing the
Church.
5. That the Company being a non-stock, not-for-profit organization is tax-exempt under Sec.
30 (E) of the National Internal Revenue Code. As such, all of its revenue and assets
actually, directly and exclusively for religious and charitable purposes are exempt from
taxes and duties. Also, all grants, endowments, donations or contributions used for
religious and charitable purposes are exempt from tax.
