SUBSCRIPTION CONTRACT

KNOW ALL PERSONS BY THESE PRESENTS:

This subscription contract is made and entered this 20 th day of April 2021 at ADDRESS
Philippines by and between:
COMPANY NAME. a corporation organized and existing under the laws of the Philippines with
principal office at Rm. 811, Vicente Madrigal Building 6793 Ayala Ave. Makati City

WITNESSETH: THAT
WHEREAS, the Corporation has an authorized capital stock of TWO MILLION
PESOS (Php 2,000,000.00).
WHEREAS, the Corporation has offered to sell shares of stock from the said increase of
capital and the Subscriber is willing and ready to buy the same.
NOW THEREFORE, in consideration of the foregoing and the subscription price
indicated hereunder:

2. Of the subscription price, the Subscriber has paid to the Corporation the amount of ONE
MILLION FIVE HUNDRED PESOS (P 1,500,000.00) in CASH.
3. The said paid-up was duly received by the Treasurer of the Corporation.
4. The parties hereby confirm the foregoing, upon the signing of this subscription contract.
