BEHAVIORAL GOALS AND OBJECTIVES:
State new students behavior to be encouraged:
Jack will use appropriate strategies when he becomes frustrated to handle
situations using positive behavior.
Describe three actions the student will demonstrate to illustrate this
behavior:
1. Be respectful.
2. Be positive, not making negative comments or showing negative body
language
3. Non-argumentative
Monitoring Process:
Jack will set goals with his special education support system regarding
goal behavior and attitude.
IDENTIFY RECOMMENDED MODIFICATIONS IN PHYSICAL
ENVIRONMENT, CURRICULUM MATERILS, INSTRUCTIONAL
STRATEGIES AND/OR TEACHER RESPONSE:
1. Physical environment
Jack will be placed near teacher in the classroom to monitor behaviors
and reinforce positive behavior.
2. Curriculum materials
Modifications can be made to assignments or tasks to Jacks academic
levels.
3. Teacher response
Teachers will respond in a positive manner, reinforcing positive behavior.
Teachers will verbally remind Jack of his positive comments and his
strengths. Rules and consequences will be clearly identified to Jack.

CLASSROOM:
Reinforcers
Jack will receive positive verbal and nonverbal cues to reinforce positive
behavior. Positive praise goes a long way with Jack.
Consequences
Loss of rewards: Teacher helper, computer time, lunch buddy w/teacher
HOME:
Reinforcers
Parent will reinforce and stay in communication with teachers at school.
Consequences
Loss of phone and video game privileges.
GENERAL PROCEDURES TO BE FOLLOWED BY:
A. Special Education Teacher
Special education teacher will reflect daily (check-in) with Jack on behaviors
and will communicate with teachers and parents.
B. General Education Teacher
Teacher will communicate with special education teacher and parents. They
will also reinforce positive behavior.
C. Administration
Administration will be notified of behavior plan.

REVIEW:
Plan will be reviewed by February 17, 2013 or upon a disciplinary action
resulting in suspension.
