Submission of medical check-up report

We kindly ask you to submit the medical check-up report when joining Rakuten. This is
obligated by the Ministry of Health, Labor and Welfare for a use in reasonable step of hiring
and health control after joining. Please submit the medical check-up report before joining
Rakuten. The medical check-up report must be taken within 3 months before the date of
entrance, including the following items.

Ordinance on Industrial Safety and Health Article 43

Item of medical check-up

Investigation of anamnestic history and occupational history

Examination of subjective and objective symptoms
Examination of height, weight, eyesight, and hearing
Examination of X-ray of chest
Measurement of blood pressure

In the medical check-up

Please send the result of the medical check-up report by following an

instruction which will be provided by mail from Labor & HR Group.
When there is not enough time between the job offer and the entrance date, the instruction
will be handed on the first day of your joining.
