    Care Beyond The Boulevard

    Computer Tablet Agreement

    User Responsibility:
    All Computer users are responsible for the safe-keeping of the computer tablets. All
    reasonable precautions should be taken to secure the tablets to prevent loss, theft or
    breakage.

    Specific machines will be assigned to and labeled for staff members


    MAs, students and others will share tablets, and will check them out and back in
    daily.
    Any and all changes and/or data entered while a user is logged on to their
    specified computer tablet will be attributed to that user.
    It is required to log off the tablet/work stations when not in use.
    All FHC computers are for official work only: no private use is allowed on these
    computers.

    Volunteer liability:
    Loss of or damage to checked out computer tablet, resulting from carless
    disregard of these security procedures, may result in responsible individual
    being required to replace the computer tablets ( at a cost of approximately
    $1,500.00).
