CLB Students Tablet Agreement

The tablet is the property of Colegio ng Lungsod ng Batangas (CLB) and issued to
student for the purpose of conducting distance learning. It is intended only for the use of
the student to whom it is assigned. The tablet can be taken home or to other locations
after school hours by the student. However, the student is responsible, at all times, for
the care and appropriate use of the tablet. CLB will have 24/7 access to devices and
digital content. Each tablet is equipped with security software. It is essential that the
student notifies the IT Office immediately if the tablet is lost or stolen.

The tablet is configured to be used on the school network. The IT Office will not be able
to assist you at your home in order to connect the tablet to other internet providers. The
tablet is issued to you for the purpose of distance learning. If you transfer to other
schools, the tablet will be reassigned to other student. The tablet will need to be
returned to the IT Office from time to time to receive regular maintenance and upgrades.
You will be notified when this becomes necessary. City government and College
policies regarding appropriate use, data protection, tablet misuse, health and safety
must be adhered to by all users of the tablet.

As a borrower of CLB tablet, I accept the following responsibilities:

1. I will follow the guidelines listed below for proper care of the tablet.
2. I will use the tablet for distance learning purposes.
3. I will not install any software on the tablet unless it has been approved by the IT
Head (Requests for software modification or installation should be made 3 days
in advance when they are needed.)
4. I will not write on or place any labels or stickers on the tablet.
5. I will not disable or uninstall the virus protection program that is provided with the
tablet.
6. I will report any problems/issues I encounter while using the tablet to the IT Office
immediately.
7. I will ensure any documents I create will be moved from the tablet to the network
on a monthly basis for backup purposes.
8. I understand that the IT Head will reimage the tablet at any point when it
becomes unusable or unstable and at the end of each semester.
9. I understand that reimaging may be a course of action for any repairs or
modifications on the tablet and this will result in the loss of all data from the
tablet.
10. Any modifications I make in the tablet’s settings will be for usability or cosmetic
reasons only.
11. All tablets must be returned at the end of each semester for inventory and
software updates. Tablets will be reassigned as deemed appropriate by the
administration.

Guidelines for Proper Care of the Tablet:

1. The tablet is not to be loaned to anyone.
2. Other individuals, including household members, should not be allowed to play
on the tablet.
3. Proper care is to be given to the tablet at all times, including but not limited to the
following:
a) Give care appropriate for any electrical device.
b) Use a surge protector or unplug the tablet during electrical storms.
c) Keep food and drink away from the tablet.
d) Do not leave the tablet exposed to direct sunlight or extreme cold.

e) Position the tablet on a safe surface so it does not drop or fall.

f) Do not attempt to repair a damaged or malfunctioning tablet.
g) Do not attempt to upgrade the tablet or software.
4. Proper security is to be provided for the tablet at all times, including, but not
limited to, the following:
a) Secure your tablet in a safe place at the end of the day.
b) Do not leave the tablet unattended.
c) Do not leave the A/C adapter behind when moving the tablet.

I understand that all tablet, equipment, and/or accessories that the school has provided
to me are the property of Colegio ng Lungsod ng Batangas. I agree to the terms
outlined in CLB Students Tablet Agreement. I understand that I will report any damage,
loss, or theft of the tablet to the IT Head.

Additionally, I understand that I will not be held responsible for tablet problems resulting
from regular school-related use; however, I understand that I am personally responsible
for any damage, theft, or loss of the tablet and/or related equipment and accessories
due to negligence. I understand that a violation of the terms and conditions set out in
the CLB Students Tablet Agreement will result in the restriction and/or termination of my
use of CLB tablet, equipment, and/or accessories and may result in further discipline up
to and including termination of enrollment and/or legal action.
