iPad/Dell Program Parent Agreement Form
Technology in the Worthington School District has been integrated to help prepare
students in a rapidly changing world. Students proper use of technology is a valued resource to our community. All electronic technologies must be used to support the educational programs in the District. Access to the Districts technology
may be revoked at any time for abusive or inappropriate conduct related to the use of digital technologies.
Failure to comply with the Districts Internet Use and Safety, and Electronic Communications Policy
(524), Bullying Prohibition Policy (514), or the guidelines stated in the Tablet Implementations Guide
may result in the loss or privilege to take the tablet home or use of the tablet in general.
The tablets are the property of Worthington Public Schools and as a result may seized and reviewed at
any time. The student should have NO expectation of privacy of materials found on a school owned
tablet.
I have read all the policies and guidelines in the Worthington Public Schools Tablet handout and understand my
responsibilities as a student in the tablet program.

Optional Tablet Insurance
Insurance is available for all District tablets. Families who purchase insurance after receiving a tablet must have the tablet
inspected by tech staff in order to verify the tablet is not damaged at the time of insurance purchase. Insurance will cover
the school year, from date of inspection after purchase until the end of the school year. The annual cost of the insurance is

CORDS, BRICKS, AND CASES ARE NOT COVERED UNDER THE INSURANCE PROGRAM
