STUDENT/PARENT CONTRACT

Due to the uniqueness of the online learning program, certain standards and behaviors are
expected of students.

As a student enrolled in an online learning modality, I am aware that:

1. I will attend the online orientation prior to beginning my online classes.


2. I understand that once enrolled in a fully online class, I must have a stable internet
connection and a digital device (e.g. laptop, desktop, cellphone) to use during online
classes.
3. I will regularly attend classes, participate fully in class discussions and activities, and
comply promptly all the requirements such as: quizzes, assignments, activities, projects,
and outputs.
4. I will inform my teacher in a timely manner if I am experiencing difficulty (e.g. technical,
navigational, or comprehension difficulties).
5. I will inform my teacher adviser or the Principal’s Office if I am sick or of any scheduled
or unscheduled absences and provide an excuse letter in order to take make up missed
work as required.
6. I will properly wear complete uniform with ID and adhere to the prescribed haircut for
boys and no hair color for girls.
7. I will always open the video camera of my digital device during online classes.
8. I will exhibit appropriate netiquette while communicating with my peers and teachers.
9. I agree to follow the examination schedule (quizzes, long tests and major exams) that my
teacher and school provide me.
10. I will respect my teachers and my classmates and should not be a bully.
11. I will support and cooperate by attending all university and Junior High School activities
such as: convocations, Christ Emphasis Week, University Day, etc.
12. I will inform my parents about our scheduled examinations and card days, and likewise
give them all the communications/letters sent by the school relative to my studies.
13. I will be committed to my studies in order to be promoted to the next level at the end of
the school year.
14. If I am found not making any improvement in conduct and class performance in any
grading period; if I have received two (2) or more failing grades in any grading period
and if I fail to meet any of the above-mentioned stipulations, I am willing to transfer to
another school or be dropped from the school rolls.
15. I must follow all other rules as specified in the CPUJHS Student Handbook, and I will
not violate any of the rules and regulations of Central Philippine University and the
Code of Conduct of the University Junior High School.

and helping him/her create and maintain a study schedule throughout the year.
□ I agree to be accessible and readily available to the teacher/adviser to discuss my
child’s progress and development.
□ I understand time management is vital to my child’s success.
□ I agree to set up a study space, including technology and stable internet connection
required for my child.
□ Failure to follow these policies may result in dismissal of my child from the class roll.
