ONLINE DISTANCE LEARNING CONTRACT

The contract terms below ensure that you are aware of the expectations, policies and
guidelines in taking online mode of learning. By signing this contract, you are acknowledging
awareness of your responsibilities for completing the requirements set by the school.

Students

Synchronous Class

1. Students are expected to uphold the Ignacian Marian values; thus, shall project proper
behavior during online learning instructions.
2. Students must have their own space that is conducive for learning. Such spaces should be
comfortable, safe, and free from interruptions or hazards.
3. Students are required to wear their school or PE uniform during the synchronous class.
They must have the prescribed hair (haircut and haircolor) and must not put on make-up.
4. Students should enter their class five minutes before it starts. They are required to have a
reliable internet connection and other gadgets which are vital for online learning.
5. Students must turn on their cameras during the entire session.
6. Students must always mute their mic until they are going to ask or answer questions.
7. Students should avoid doing other tasks, checking emails, playing games, opening other
sites and looking at their phones while their classes are still on-going.
8. Students should understand that no make-up policy for the missed activities and
performance tasks. Unless their reason for not answering activities is valid and certified
by their parent/guardian.
9. Students must also check email or group chats regularly for messages or updates from the
adviser/subject teacher. It is the student’s responsibility to contact the subject teacher if
they are experiencing difficulties in completing the task.
10. Students should understand that failure to meet any of the minimum competencies and
requirements will result in a failing grade.
11. Students are prohibited to record the teacher's discussion unless they are told to do so.

Shifting of Modality

Shifting of modality is prohibited.

Attendance

1. Students’ attendance for the morning session will be checked by the teacher during the
Google Meet.
2. Students’ attendance for the afternoon session is based on their submitted output.
Students must complete the activities on or before the deadline for them to be able to be
marked present.
3. If students fail to submit their output on time, he/she will be marked late. However, if the
reason for the delay is valid, it might be considered provided that the reason is valid and
the subject teacher was informed.
4. Students who consistently arrive late during class hours without valid reason or proof, are
required to submit an explanation letter to their adviser stating the reason of his/her
tardiness.

Examination

1. Exams are scheduled every Friday and Saturday of the grading period. Students are
required to take and submit their exams according to time allotted for a particular subject

2. Students who are absent during the exam day should submit an excuse letter stating the
reason why they failed to take the exam. After submitting an excuse letter, the subject
teacher will allow the student to access the form.

Parents/Guardians:

1. Parents should provide students with a conducive learning environment. It should be well
ventilated, quiet, and comfortable for the students.
2. Parents must be aware of the class schedule for them to be able to monitor the student
well.
3. Parents must ensure that the students are seated, connected and ready to join the online
class five (5) minutes before the time.
4. Parents must be aware of the online requirements. They must monitor work habits and
progress, and ensure that students will only access appropriate online portals. Parents
should also become familiar with digital platforms your child is using and ask your child
to show you his/her progress in every subject.
5. Parents should be aware of the deadlines, schedule of exams and submission of activities.
6. Parents should also be in frequent contact with the teacher and inform him/her of any
concerns with the subject or learning problems that arise.
7. Parents must ensure that the given schedule will be religiously followed by the students.
8. Ensure that all tests and work are completed by your child, that the work turned in by the
student is the student’s work, and that it is not done by or aided by others. Keep track all
the deadlines that your child need to finish and accomplish.
9. Parents should inform the adviser or subject teachers on the difficulties their child is
experiencing.
10. Parents must communicate with the adviser through their mobile numbers during office
hours about the progress of your child as needed.
11. Parents must fully acknowledge that success is largely dependent upon the work and
effort put forth by the student.

By signing this contract, both the parent and the student agree that they understand
the requirements set forth above; that they will meet the listed expectations; and that they
understand that the student’s success depends on their meeting the expectations.

MODULAR DISTANCE LEARNING CONTRACT

The contract terms below ensure that you are aware of the expectations, policies and
guidelines in taking modular mode of learning. By signing this contract, you are acknowledging
awareness of your responsibilities for completing the requirements set by the school.

Students

Completing the Modules

1. Students are expected to read all the learning materials in the modules.
2. Students are responsible to contact their subject teachers if they do not understand the
learning materials or if they are having a hard time meeting the subjects’ requirements.
3. Students must complete on their own the activities or worksheets and performance tasks
found in the module.
4. Students should understand that no make-up policy for the missed activities and
performance tasks. Unless their reason is valid and certified by their parents/guardians.
5. Students should be aware that plagiarized outputs, will have no merit and they will be
subject to proper disciplinary actions as stipulated in the Student’s Handbook
6. Students should understand that failure to meet any of the minimum competencies and
requirements will result in a failing grade.

Shifting of Modality

1. Shifting of modality is prohibited.

Attendance for Modular Learning Instruction

1. Student’s attendance is based on the submitted worksheets.


2. Students whose worksheets are consistently submitted, in advance or on time, will have
perfect attendance.
3. Students whose worksheets are not consistently submitted on time will have points
deducted from their attendance. Below is the ratio of the number of days late and its
equivalent absences.
4. Early or on time submissions is equivalent to no absences.
1 day late is equivalent to 2 absences
2 days late is equivalent to 4 absences.
3 days late is equivalent to 6 absences
5. Modules submitted 4 days after the deadline will not be considered. Conference with the
parents will be scheduled. However, if reasons are valid with proper communication,
modules will still be accepted.

Exams
1. There are four quarterly examinations for every semester.
2. Modules must be submitted first before getting the examination papers.
(Applicable for JHS and SHS only)
3. Students should understand that no make-up policy for the missed exams. Unless their
reasons for not taking the quarterly assessment are valid and certified by their
parent/guardian.
4. All test papers will be released on the first day of exam at 8:00a.m. and will be returned
two days after it has been released.
Example:
Exam: September 1 to 2
September 1- Releasing of Exam Papers
September 3- Returning of Exam Papers

Parents

1. Parents must co-supervise and co-monitor the progress of their child’s learning. If the parent
cannot supervise, the student is not allowed to enroll in modular classes hence he/she is
advised to transfer to online mode of learning.
2. Parents are asked to communicate with the school through formal writing stating the reasons
for their failure to submit the worksheets and examination papers on the deadline.
3. Parents are required to bring flash drives for the softcopy of the modules if they will print the
module on their own.
4. Parents should consult with the teachers on the challenges met by their child.
5. Parents are responsible for getting and submitting the learning modules.
6. Students are not allowed to get or submit their modules unless their age is included in the
guidelines given by the IATF-Davao.
7. Once the student and the parent decide that they prefer modular distance learning, shifting to
another modality is not allowed.
