Sod aoe pee April 26, 2010 Senior (Grade 8/9) CSS Track and Field Day - Glenmore Athletic Park ‘The Grade 8 and 9 classes will be participating in our PE Field Trip this year at the Glenmore Athletic Park on Monday, May 3, 2010. We will be departing, by bus, from CSS at 8:30 am and returning at 3:00 pm from Glenmore Athletic Park. Students. need to be at CSS for 8:15 am. There will be a BBQ lunch provided for the students at the track, but they are welcome to bring snacks, water and/or sports drinks. Due to the healthy living values that we try to instill in the students, junk food (pop, chocolate bars, chips, other empty calorie foods) will not be permitted. The Field Trip is a component of our PE curriculum and attendance is mandatory. Doctor's and/or parental notes will be required in the event that a student cannot physically participate (they can volunteer at stations.) Please let your child’s homeroom teacher know if you can volunteer for the day. (Description of activity and dates) ‘THIS FORM MUST BE READ AND SIGNED BY EVERY STUDENT WHO WISHES TO PARTICIPATE AND BY A PARENT OR GUARDIAN OF A PARTICIPATING STUDENT. ELEMENTS OF RISK: [Educational activity programs, such as Track and Field Days involve certain elements of risk, Injuries/or death may occur while participating in these activities. The following list includes, but is not limited to, ‘examples ofthe types of injury which may result from participating in Track and Field Events. Strains, sprains, other soft tissue injuries Bone breaks Heat related illnesses or injuries (e.g. heat stroke, sunburn) vehicle accident in transport “The risk of sustaining these types of injuries result from the nature of the activity and can occur without fault of either the student, or the schoo! board, its’ employees/agents or the facility where the activity is taking place. By choosing to take part in this activity, you are accepting the risk that you/your child may be injured, “The chance of an injury/or death occurring can be reduced by carefully following instructions at all times while engaged in the activity.
