Incident Report on (GCI Highway) Warehouse under Sir. Perez,

Last 14th of March 2016 at around 2:40pm in the afternoon, Mr.
Vincent Malabanan poured an amount of Nipicide to the container
bottle and weighted it, then suddenly he noticed that it was
underweight so he added an another amount of Nipicide on the same
container bottle. He didnt noticed the said chemical was already
spilled to the lower left portion of his pants. After a few minutes, he felt
itchy and a burning sensation on the spilled area of his pants. As he
got home, there were already blisters around his left foot.
