2. Can Identify numbers: Yes: Child identified numbers 1-10 when I asked her to put objects
matching the number: put 5 buttons in number 5 she was able to identify each number.

with a circle, a refrigerator she matched with a rectangle, a box with a square, and a

23. Can recognize difference in size: Yes: When doing high and weight child said, I am
taller than her.
24. Shows interest in books: Yes: Child brought a book to me and said, teacher can you

25. Can retells the main idea of a story: Yes: We read a book about recycle and I asked
what was the book about, child said, It is about keep the environment clean.

child put the pieces in order, first the seeds, watering, and tree.

pattern, red, green, blue, she followed the pattern three times.

carpet and child followed the directions, putting a puppet in different places, next to,
behind, in front, and top.

words.
Social & Emotional Development:
1. Shares materials: Yes: Child was playing with play dough and another child wanted to
have play dough too, and V. cut half and gave it to the child.
2. Takes turns: Yes: Children were going down the slide and V. waited for her turn
staying in line.

3. Shows respect and rights of others property: Yes: When playing outside child grabbed a
toy from another child, and she gave it back when the other child let her know that she was
using it.
4. Keeps hands to his/her self: Yes: When lining up to go outside child kept her hands
down without touching anything around her.
5. Listens while others speak / tell a story: Yes: When I was giving directions about fire
drill V.M. was listening.
6. Is polite / courteous to other children & teachers: Yes: V.M. asked a child next to her if
he needed help to open the milk carton.
7. Helps classmates clean up when play time is over: Yes: When it was time to clean
up, and she heard the clean song, she started putting toys away.
8. Can attend to bathroom needs by his/her self: Yes: Child used the bathroom, wash
and dry her hands by herself by letting me know when she needs to use the
bathroom.
9. Makes attempts to put on his/her own coat, jacket, and sweater: Yes: It was time to go
outside child grabbed her jacket from her cubby and put it on by herself.
10. Can zip, snap, button clothing: Yes: Child zipped her jacket by herself.
11. Separates from parents with ease: Yes: Child waved her mom bye at arrival time and
walked to wash her hands.
12. Seeks other children to play with: Yes: Child asked another child if she can ride the
wagon with her.
13. Can express anger in words rather than actions: Yes: Child expressed frustration
when other child step on her sand castle , she said, you cant step on my castle,
now I have to make it again. Child was able to use her words instead of action.
14. Smiles, seems happy much of the time: Yes: At arrival child said smiling, good
morning teacher.
15. Works and plays cooperatively with other children: Yes: V.M. joins a group of children
playing with sand, and she plays cooperatively.
16. Seeks help when needed: Yes: Child asked for help when she couldnt open the
glue bottle, she said, teacher can you help me open the glue bottle.
Work Habits:
1. Expresses ideas with art materials: Yes: Child makes a pot with play dough and
said, Teacher can I paint the pot with different colors?
2. Participates in & enjoys group singing / games: Yes: We sat on the carpet to play
pass the bean bag game, child participated by passing the bean bag and follow
the songs directions.

3. Participates in classroom activities: Yes: We sang Old Macdonald had a farm using
props and child hold the prop and said the sound of the animal when it was her
turn.
4. Follows Directions: Yes: At arrival I asked V.M. to get her name tag and pencil and
practice her name on the paper that on top of the shelf and she was able to follow
the directions of two or more steps.
5. Cares for School Materials: Yes: Child washed and put the brushes away when she
finished painting.
6. Handles scissors properly: Yes: Child holds the scissors properly to cut following the line
holding the paper with hand and using the scissors with the other hand.
7. Holds pencil properly: Yes: Child uses a three finger grasp when holding the pencil to
write her name.
8. Can trace & cut specific patterns: Yes: Child follow the block pattern using three
colors, red, green and blue. She followed the pattern three time.
9. Can paint & paste neatly: Yes: Child made a yellow collage gluing different
materials.
10. Attempts to color within the lines: Yes: Child drew a circle and color inside the line.
11. Completes tasks: Child chose a puzzle and tried the pieces until she founded all
the right pieces.
12. Shows interest/ attention in classroom activities (appropriate attention span) Yes: Child
was able to listen when I was giving directions about the fire drill.
Large Motor Development:
1. Runs with control over speed and directions. Yes: When the child said, its dinner
time V.M. run back to the fence.
2. Jumps over obstacles, landing on 2 feet: Yes: Child jumped off a low landing on two
feet.
3. Climbs up and down climbing equipment with ease: Yes: Child climbed up and down
on playground equipment.
4. Moves body creatively to music: Yes: We were dancing using scarfs and V.M.
moved the scarf following the song rhythm.
5. Is willing to exercise: Yes: We dance using a song with different movement and
child was able to follow the directions.
6. Jumps on two feet: Yes: Child put hula hoops next to each other and she jumped the

hulas falling on both feet.


7. Hops on foot at least 6 hops: Yes: V.M. was able to hop 5 times without holding on

anything.

8. Can gallop: Yes: I played the song that gives different movement like hop, gallop,
skip and child was able to make all those movements.

11. Can catch a ball: Yes: I threw the ball to V.M. and she catches using both hands.
12. Can bounce catch a ball: Yes: Child bounced and catches the ball and tossed it at the

basketball hoop.
Language:
1. Speaks in complete sentences: Yes: In conversation I asked child what ice cream

flavor she likes, she said, I like strawberry ice-cream.


2. Speaks clearly: Yes: Child speaks clear and with full sentences, she said, actually
my dad is not at home, he is working, but my mom is at home she is not working.
3. Takes part in conversations with other children: Yes: Child started a conversation by
saying, I went to the park with my mom and other child said, I went to the park too,
and V.M. said, I didnt see you. They exchange conversation.
4. Expresses needs and wants clearly: Yes: Child is able to express her needs and

wants, she said, teacher I want to paint and teacher I want to take my jacket off I
am not cold anymore.
5. Dictates description of drawings: Yes: Child drew a picture and said, These are

Indians and this is a horse and a child riding it.
