Shellisa will need to develop and grow as a Social Worker and gain more experiences and
knowledge overall. She does make tense/grammatical mistakes in her writing at times, but is

Rating Scale
Ratings are distinct for each semester; they are not cumulative. Under each competency, specific
practice behaviors are listed, along with specific measures for evaluating student performance.
This form may be personalized by adding comments after each section. If any item within a
section has been rated either Exceeds, Does not meet or Marginally meets Expected
Competency, a specific comment must be included regarding that item in the area provided. In
addition, strengths, achievements and initiatives shown by the student in the performance of
his/her assignments and work contacts should be noted in the comment sections or narrative
summary.
The Rating Scale is:
(4) Exceeds Expected Competency: The student demonstrates knowledge and ability above
the criteria set for each practice behavior.
(3) Meets Expected Competency: The student demonstrates both ability and knowledge
which meet the basic expectations as listed in the criteria for each practice behavior.
(2) Marginally Expected Minimum Competency: The student requires intensive
redirection. The student demonstrates effort but has difficulty in achieving required
expectations for each practice behavior.
(1) Does Not Meet Expected Competency: The student is unable to perform even minimal
tasks and does not demonstrate an understanding of the theoretical base of materials.
(0) Not Applicable/No Opportunity: Student has little or no opportunity to engage in task.

Comments: Shellisa has developed a good relationship with her Social Work team members.
She dresses professionally and acts accordingly while in her internship. Shellisa is a mature
intern. She has taken the initiative in several ways during her internship at CST- including
going to other Social Workers and assisting with various research based needs, developing an
Ethics presentation to present to the Social Work team during a clinical meeting, and coming in
on her days off to assist when there was a need. Shellisa has great time and attendance, which is
much appreciated. This is the reason for the high score on #11 above.

Comments: Shellisa was approached by her internship supervisor about researching and jointly
facilitating a presentation on Ethics to the Social Work department for National Social Work
Month. She jumped at the idea, researched it along with her fellow intern, and presented to the
team and her Faculty Field Liaison. She did a great job! This is the reason for the high scores in
this area.

Comments: Shellisa writes case notes for every patient and community resource interaction.
Shellisa does make tense/grammatical mistakes in her writing at times, but is open to learning
and discussing it.

Comments: Shellisa approaches her fellow Social Workers for resources and goes to them and
this writer often for assistance.

Comments: Shellisa has researched and presented a training on Ethics to the Social Work
Department. She seems to understand the importance of research evidence. She wanted to make
sure that the resources were sited correctly and shared during her presentation.
VII. Core Competency: Apply knowledge of human behavior and the social
environment
Practice Behavior: Environment/Assessment
Comments: Shellisa advocates on behalf of her patients while using cultural sensitivity and
understanding.
Comments: During her internship, Shellisa has attended weekly clinical meetings, where we
discuss agency policies and procedures. She has developed an understanding of various
collaborations, such as AAA-1B, in which she watched a webinar and heard a leader within the
company present on the collaboration CST has with them.

certain services
Comments: Shellisa has been working on developing a national durable medical equipment
resource data base. During this time, she has been made aware of the medical trends within our
country and how they effect our patients/the elderly.

Comments: Shellisa has appropriate interpersonal interactions with both her patients and peers.
She displays empathy for the patients she assists and they seem to develop good relationships
with her. This is evident in her Process Recordings and during her recorded phone calls we
listen to and process together during supervision.

Comments: Shellisa took an active role in the development of her Learning Plan. She is
prepared and an active participant in our supervision sessions and weekly clinical staff
meetings. Shellisa did not interact with data too much during her internship, but she receives
data reports that are reviewed. Data has also been a topic during clinical meetings.

Comments: Shellisa advocates for the patients she works with. She has not had the opportunity
to participate in transitions and ending directly with patients in a therapeutic way, due to the
nature of her internship. She has had the opportunity to see how patients transition in and out of
various services and how their needs change as a result. She has also had team members
transition to new employment opportunities and has seen the effect of those changes on the
Social Work team, patients, and company as a whole. She has even been offered employment at
CST until her graduate program starts to assist with these staff transitions.
