A. Registration Requirements:
Interested contractors with qualifications and classifications related to the scope of work being bid, must submit the
following documents during the Registration period:
1. Original letter on contractor s letterhead paper, with clear address and contactable telephone, fax numbers & e-mails,
requesting participation in the Bid.
2. If contractor formed as a consortium, contractor shall clearly state and register as a consortium.
3. SIUP, NPWP and PKP (contractor is to show the original documents at the time of registration).
4. Copy of domicile letter.
5. Copy of Company s deed and its ammendment (if any) with letter from the Minister of Justice approval.
6. Statement Letter to confirm Domestic Content Criteria (40 %).
