EQUIPMENT REPAIR /MAINTENANCE FORM

TO ALL OF THE DEPARTMENT:

1. This form is version 2 of a trial to streamline the repair and maintenance of equipment.

2. In future all equipment that needs to be maintained or repaired, ( or even if you see trends happening)
must first have this form filled in.

3. Make copy of form for your records. Lodge original with Alida.
(a) Initiate action yourselves, then get serviceperson to fill out form (in their spot) and lodge
filled original form with Alida.
(b) For Alida to initiate action, lodge original with her (make note that you have not initiated
any action yourself).

4. REASON FOR FORM.


Equipment is taken away for repair and there is no record of removal.
Generally our departmental staff do not know what is happening to communal equipment. These forms
will allow transfer of knowledge to all levels.
Trends can occur with equipment long before we get total breakdown.
Total breakdown equals lost research time and the expenditure of possibly lots of $$$$$$.
Knowledge of equipment playing up or knowledge that equipment will be removed may allow you to
rearrange your experiments.
We have no means of tracking what is happening to our equipment (other than in the minds of Dieter
and /or John).
