Dear Sir:

With sincere acknowledgment of the support of the College, I am honored to be part of the 13
candidates from Region III who subjected themselves to this year’s second group for
Professional Accreditation, and who fortunately passed the examination. This was held last
August 20 at the Central Luzon State University, Munoz, Nueva Ecija.

In this regard, may I request that I be given the benefit provided for by the BASC-BOT
Resolution No. 10-690, approved on June 10, 2010 at BASC, to wit:

“Approving the authorization for the College to assume fifty per cent (50%) of the
Professorial Accreditation fee for qualified members commencing 2010 and the
years thereafter.”

Enclosed is the report of ratings for your reference.

I reiterate my deep appreciation and gratitude for all the support you have been extending all of
